Reviews
Under the Care Act, you are entitled to an annual review of your care plan. Unfortunately, due to resource constraints, Local Authorities often face delays, sometimes stretching reviews to 2 or even 3 years. With my extensive experience conducting care reviews, I can ensure your care package is evaluated in a timely, person-centered manner. I focus on identifying changes needed, celebrating your strengths, and aligning support with your evolving needs.
Carer’s Assessments
As a carer, your needs matter too. My assessment process focuses on understanding the impact of your caring responsibilities on your health, wellbeing, work, leisure, and personal identity. Together, we will identify the support you need to maintain balance in your life and access resources that enable you to thrive.
